Job summary

Jobtailor is seeking an LPN/LVN to provide in-home nursing care aligned with individualized patient care plans. You will review Plans of Care before visits, report updates to the care team, and assist with medication management and documentation.

Qualifications

• Current LPN/LVN license in the applicable state.
