Job summary

Jobtailor in Rhode Island seeks a Licensed Vocational Nurse to join our hospice and home health team, delivering patient-centered care in close collaboration with RNs and physicians.

You will administer medications, update care plans, and educate patients and families to support self-care, while working with the RN Case Manager to adjust plans as patient needs evolve. This role emphasizes compassionate, proactive care to improve outcomes.

Qualifications

• Current State LVN license in Rhode Island.
• Graduate of an accredited nursing program (certificate/diploma/associate).
• At least 1 year of clinical care experience.
• Ability to lift up to 50 pounds and assist with patient positioning.
• Hospice or home health experience preferred.

Responsibilities

Requirements

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Experience in a hospice or home health environment is preferred
