Job summary

Responsibilities

• Assist in gathering data to assess physical, psychological, social, and spiritual needs of patients.
• Collaborate with the RN and interdisciplinary team in planning for patient needs on admission.
• Review care plans and provide input on updates.
• Support direct patient care using nursing skills and judgment.
• Educate patients and families and document outcomes in the medical record.
