Qualifications

• Ability to interact effectively with a multi-disciplinary staff and patients.

Responsibilities

• Triages patient phone calls following established guidelines and data collection formats.
• Documents health information, interventions, and outcomes into the EMR.
• Provides health education to patients based on identified needs to promote health.
• Administers prescribed medications according to the five rights of medication administration.
• Performs physical and psychosocial assessments and plans nursing care accordingly.
• Completes unit-specific clinical skill competencies annually.
• Performs other nursing services commensurate with LPN experience and competencies.
