We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. As an LPN, you will be responsible for providing high-quality, patient-centered care to residents and ensuring a safe and supportive environment. You will play a vital role in enhancing the quality of life for our patients by delivering excellent clinical care, managing patient interactions, and collaborating with other healthcare professionals.

Essential Duties

• Administer medications
• Monitor and assess patient conditions and promptly report changes
• Assist in patient admissions, discharges, and transfers

• Maintain accurate patient records and documentation in accordance with facility policies
• Coordinate with doctors, CNAs, and other healthcare team members to deliver comprehensive care
• Educate patients and families on health management and discharge instructions

Work Experience Requirements

• Current LPN license in Kentucky

• Ability to work in a fast-paced environment and handle challenging situations with grace
