Job Overview - Licensed Practical Nurse (Part-Time):
Our client in Brooklyn, NY, is seeking a Licensed Practical Nurse (LPN) to provide medication management, health education, and psychoeducation. This part-time role delivers 80% of services in the community, adhering to ACT principles and nursing ethics.

Compensation: $40/hour
Location: Brooklyn, NY
Schedule: 17.5 Hours/Week

Responsibilities as the Licensed Practical Nurse (Part-Time):

• Community-Based Care: Provide 80% of medication management and health services in the community, including administering medications, monitoring side effects, and coordinating healthcare services.
• Documentation: Complete and sign progress notes, incident reports, and other documentation in compliance with OMH regulations, DOHMH guidelines, and Medicaid billing standards.
• Coordination: Work with FACT RN, prescribers, and other service providers to manage and coordinate consumers’ health care and medication schedules.
• Collaborative Care: Coordinate care with family members, hospitals, housing providers, and other community service providers.

Qualifications for the Licensed Practical Nurse (Part-Time):

• Licensure: Current New York State Licensed Practical Nurse (LPN) license.
• Experience: 3+ years of experience working with adults in psychiatric nursing, substance use treatment, or social service settings.
• Skills: Highly organized, self-motivated, and proficient in computer usage.
• Language: Fluency in Spanish is preferred!
