Concordia Visiting Nurses Cabot is seeking Licensed Practical Nurses with Wound/Ostomy certification for full- and part-time home health positions covering parts of the Butler area. This role requires PA licensure and reliable transportation.

You will provide skilled nursing services in patients' homes, coordinate care with the broader team, and meet agency standards in a supportive, patient-centered environment, with weekday hours and occasional weekends.

Qualifications

• Current license in the State of Pennsylvania.
• Graduate of state-approved school with course in pharmacology.
• Minimum of two years experience in patient care.
• Reliable, independent transportation for home visits.

Responsibilities

• Provide wound/ostomy nursing care in patients' homes.
• Cover parts of the Butler coverage area with 1:1 patient care.
• Comply with state licensing and Concordia policies.

QUALIFICATIONS

• Graduate of state-approved school with course in pharmacology
• Current license in the State of Pennsylvania
• Minimum of two [2] years experience in patient care
• Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state.
