Summary

Provides direct patient care, addressing psychological and physical needs according to the care plan, physician's orders, and departmental policies. Patients range from neonates to geriatrics. Care settings include inpatient, ambulatory, home health, or hospice.

Responsibilities

• Develops and maintains individualized, goal-oriented care plans, consulting with health professionals to meet patient/caregiver needs.
• Observes, assesses, documents, and reports patient responses to treatments, adjusting care priorities as needed. Collects data on biophysical, psychosocial, environmental, self-care, educational, and discharge needs.
• Participates in departmental activities such as committees, CQI groups, and staff education.
• Administers treatments including respiratory therapies, suctioning, medication administration, cardiac monitoring, assisting with procedures, IV infusions, lab work, catheter care, dressings, and patient teaching.
• Assists patients and families during admission, orientation, and discharge processes.
• Supports patients and families, de-escalates distressed or aggressive patients, and facilitates age-appropriate coping strategies.

Other Information

Education: Graduation from an accredited practical nurse program.

Licensure/Certification: Licensed as an LPN in North Carolina, BLS certification required.

Experience: No prior experience necessary.

Skills: Knowledge of clinical procedures, patient care, and communication skills.
