Summary:
Provides direct patient care, addressing both psychological and physical needs according to the care plan, physician's orders, and departmental policies. Patients range from neonates to geriatrics. Care settings include inpatient, outpatient, home health, or hospice.

Responsibilities:

1. Develops and maintains individualized, goal-oriented care plans in collaboration with other health professionals to meet patient and caregiver needs.
2. Monitors, assesses, documents, and reports patient responses to treatments and education; adjusts care priorities accordingly. Collects and reports data on biophysical, psychosocial, environmental, self-care, educational, and discharge needs.
3. Participates in departmental activities that promote positive outcomes, including committees, CQI initiatives, and staff education.
4. Administers a broad range of treatments, including respiratory therapies, suctioning, medication administration, monitoring cardiac and respiratory devices, assisting with procedures, and patient teaching.
5. Assists patients and families during admission, orientation, and discharge processes.
6. Supports patients and families by de-escalating agitation and facilitating age-appropriate coping strategies.

Other Information

Education Requirements: Graduation from an accredited practical nursing program.

Licensure/Certification Requirements: Licensed Practical Nurse (LPN) in North Carolina and BLS certification.
