Job summary

Mountain Valley Hospice is seeking a dedicated Licensed Practical Nurse (LPN) to join our team. You will provide direct patient care, including medication administration, wound care, and treatments under the supervision of a registered nurse, in patients’ homes, SNFs and ALFs.

You will collaborate with an interdisciplinary team and monitor patient conditions, communicating changes promptly to the care team. Schedule is Monday–Friday, 8-hour days from 8 AM to 5 PM, in Galax and surrounding areas.

Qualifications

• Valid Licensed Practical Nurse License
• Exceptional interpersonal and communication skills, with a compassionate and patient-centered approach.
• Ability to work independently and as part of a team, with excellent time management skills.

Responsibilities

• Provide direct patient care including medication administration, wound care, treatments and procedures under RN supervision.
• Collaborate with interdisciplinary team to address patient and family needs across home health, SNFs and ALFs.
• Monitor and document changes in patient conditions and communicate these to the care team.

What You’ll Do:

• Deliver Exceptional Care: Provide direct care to patients, including administration of medication, wound care, treatments, and procedures under the supervision of a registered nurse.
• Collaborate: Work with the interdisciplinary team to address patient and family needs, including connecting with other providers (aides, social workers, chaplains, and more).
• Monitor and Respond: Recognize and document changes in patient conditions and communicate these effectively to the care team.

Your Schedule:

• Monday - Friday, 8-hour days, 8 AM and 5 PM.
