Cooperstown Center is hiring an LPN Supervisor to work the Evening Shift in Cooperstown, NY.

Duties

1. Conduct facility rounds.
2. Monitor assignments of aides for proper workflow.
3. Supervise medication assistance times, ensuring proper recording of assistance, refusals, and follow-up.
4. Intervene in resident conflicts, outbursts, and behavior issues.
5. Facilitate communication to address medical/mental health situations promptly.
6. Complete incident/accident reports and provide summaries to EHP Coordinator/Administrator.
7. Attend to late admissions/re-admissions (paperwork, medication changes, etc.).
8. Communicate shift activity via 24-hour report for review by the EHP Coordinator/Administrator.
9. Record environmental situations and contact EHP Coordinator/Administrator as needed.

Requirements

1. Ability to work as part of a team.
2. Valid state LPN license.
3. Good standing with State Registry.
