Job summary

Hamilton Arms Center in Lancaster, PA is seeking a Full Time Licensed Practical Nurse Assessment Coordinator (LPNAC) to lead and coordinate our MDS team on site. The LPNAC will oversee assessments, RAI processes, and ensure compliance with PDPM/PPD and Medicare/Medicaid billing requirements.

Candidates should have LPN experience with strong leadership/assessment skills, knowledge of PA regulations, and be prepared for background checks and a pre-employment screen.

Qualifications

• Must be confident in MDS and resident assessments.
• Experience as LPNAC and knowledge of the RAI process.
• Licensed Practical Nurse with leadership and assessment experience.
• Knowledge of state and federal regulations (PA preferred).
• Medicare/Medicaid billing knowledge.
• Familiarity with PDPM/PPD.
• Background check and drug screen required.

Responsibilities

• Coordinate resident assessments per facility policies.
• Manage the RAI process in line with Medicare/Medicaid rules.
• Develop and maintain a QA program for resident assessment.
• Support facility surveys and regulatory inspections.
• Monitor QI/QM reports and drive corrective actions.
• Educate interdisciplinary team on the RAI process.

Requirements of the Licensed Practical Nurse Assessment Coordinator (LPNAC):

• Must be confident in MDS

• Must have previous experience as a LPNAC and assessments

• Licensed Practical Nurse (LPN) with effective leadership & assessment skills in a long-term care setting

• Must have knowledge of state and federal regulations

• Proficient in PA CMI

• Licensed Practical Nurse Assessment Coordinator (LPNAC) must have Medicare billing knowledge and experience.

• Knowledge ofPPD (PDPM)

• Experienced in Insurance updates

• Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.

Responsibilities of the Licensed Practical Nurse Assessment Coordinator (LPNAC):

• Conduct and coordinate the completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility.

• Conduct and coordinate the completion of the RAI process in accordance with current rules, regulations, and guidelines that govern the resident assessment in accordance with Medicare, Medicaid, OBRA, and other payer programs.

• Develop, implement, and maintain an ongoing quality assurance program for the resident assessment.

• Participate in Facility surveys (inspections) made by authorized government agencies.

• Monitor the Facility’s QI and QM reports to ensure that appropriate corrective action can be implemented when potential problem areas occur.

• Provide initial and ongoing education with the interdisciplinary team involved in the RAI process.
