Job summary

Panacea Health Corp in Lancaster, PA is seeking a Full Time Licensed Practical Nurse Assessment Coordinator (LPNAC) to lead our MDS team on-site. The LPNAC will oversee assessments, coordinate the RAI process, and ensure compliance with PDPM and Medicare/Medicaid billing.

The role requires strong leadership, onsite collaboration, and familiarity with PA state regulations. On-site position with no remote option, and a comprehensive benefits package is offered.

Qualifications

• Must have LPN license and MDS assessment experience.
• Knowledge of PDPM and Medicare/Medicaid billing.

Responsibilities

• Lead and coordinate resident assessment in line with state and facility policies.
• Conduct and coordinate the RAI process per Medicare/Medicaid rules and OBRA guidelines.
• Develop and maintain a quality assurance program for the resident assessment.
• Participate in facility surveys by government agencies.
• Monitor QM/QI reports to trigger corrective actions.
• Educate the interdisciplinary team involved in the RAI process.

Requirements of the Licensed Practical Nurse Assessment Coordinator (LPNAC):

• Must be confident in MDS
• Must have previous experience as a LPNAC and assessments
• Licensed Practical Nurse (LPN) with effective leadership & assessment skills in a long-term care setting
• Must have knowledge of state and federal regulations
• Proficient in PA CMI
• Licensed Practical Nurse Assessment Coordinator (LPNAC) must have Medicare billing knowledge and experience.
• Knowledge of PPD (PDPM)
• Experienced in Insurance updates
• Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.

Responsibilities of the Licensed Practical Nurse Assessment Coordinator (LPNAC):

• Conduct and coordinate the completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility.
• Conduct and coordinate the completion of the RAI process in accordance with current rules, regulations, and guidelines that govern the resident assessment in accordance with Medicare, Medicaid, OBRA, and other payer programs.
• Develop, implement, and maintain an ongoing quality assurance program for the resident assessment.
• Participate in Facility surveys (inspections) made by authorized government agencies.
• Monitor the Facility’s QI and QM reports to ensure that appropriate corrective action can be implemented when potential problem areas occur.
• Provide initial and ongoing education with the interdisciplinary team involved in the RAI process.
