Job summary

Uptown Hope Corporation in Parkville, MD is seeking a compassionate Licensed Practical Nurse (LPN) to provide individualized care for medically fragile and developmentally disabled residents in a residential setting.

Schedule is Saturday and Sunday, 8:00 AM to 12:00 AM, with immediate start for credentialed candidates. You will follow care plans, administer meds, document meticulously, and collaborate with a multidisciplinary team.

Qualifications

• Active LPN license in good standing.
• BLS/CPR certification required.
• Ability to manage multiple residents in a fast-paced setting.
• Experience with developmental disabilities in residential/DDA setting preferred.

Responsibilities

• Provide resident nursing care per plan and physician's orders.
• Administer medications and treatments per plan.
• Monitor residents and report changes to supervising staff.
• Maintain nursing documentation accurately and timely.
• Promote safe, respectful, therapeutic environment.
• Collaborate with DSPs, nurses, families and physicians.

Residential healthcare provider located in Parkville, MD has an immediate contract opportunity for an experienced Licensed Practical Nurse (LPN)

Summary:

• Seeking a compassionate, dependable, and clinically skilled LPN to provide individualized nursing care to medically fragile and developmentally disabled individuals in a residential setting.
• Schedule: Saturday & Sunday, 8:00 AM - 12:00 AM.
• Start: Immediate / as soon as credentialed.

Responsibilities:

• Provide resident care in accordance with accepted standards of nursing practice, the written plan of care, and the physician's plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Administer medications and prescribed treatments in accordance with the plan of care and facility policies.
• Monitor residents' conditions and promptly report changes to the supervising nurse or physician.
• Respond appropriately to residents' clinical and healthcare needs.
• Collaborate with DSPs, nurses, program staff, families/guardians, physicians, and other members of the interdisciplinary team.
• Follow all applicable DDA, healthcare, medication-management, safety, and agency policies and procedures.
• Maintain accurate and timely nursing documentation in accordance with physician orders, care plans, and regulatory standards.
• Promote a safe, respectful, and therapeutic environment for residents and their families.

Requirements:

• Active compact LPN license.
• Valid BLS/CPR certification.
• Ability to care for and manage multiple residents in a fast-paced healthcare environment.
