Job summary

Innovative Life Solutions is seeking a dependable Licensed Practical Nurse to provide person-centered care for individuals with IDD in Southeast Washington, DC. The LPN will monitor health, administer medications, maintain documentation, and collaborate with the interdisciplinary team to ensure safe, consistent care.

The role follows a Monday–Friday 8:00 AM–4:00 PM schedule with 40 hours per week, offering compensation of $30.00 to $35.00 per hour based on experience.

Qualifications

• Current, active, and unrestricted Licensed Practical Nurse license in DC.
• CPR and First Aid certification, or ability to obtain within timeframe.
• Completion of all required DC DDS Phase Trainings.
• Ability to pass background checks and regulatory clearances.
• Ability to meet all pre-employment, health, credentialing, and regulatory requirements.
• Working knowledge of medication administration, nursing documentation, infection prevention.
• Clear communication, confidentiality, and team collaboration.

Responsibilities

• Provide nursing services per person’s plan of care and physician orders.
• Administer medications and treatments, documenting in records.
• Monitor health status, identify changes, and escalate as needed.
• Complete nursing notes, assessments, and required records timely.
• Coordinate with physicians, pharmacies, hospitals, families, and staff.
• Support continuity of care during admissions, transitions, and med changes.
• Guide staff on health protocols and nursing instructions within scope.
• Follow infection prevention, safety and confidentiality procedures.
• Maintain reliable attendance for the complete shift.

Position Summary

Innovative Life Solutions is seeking a dependable and compassionate Licensed Practical Nurse to provide person-centered nursing support to individuals with intellectual and developmental disabilities. The LPN will monitor health needs, administer medications and treatments, maintain accurate documentation, communicate changes promptly, and collaborate with the interdisciplinary team to support safe and consistent care.

This position is assigned to the Monday through Friday 8:00 AM to 4:00 PM schedule in Southeast Washington, DC. Applicants should apply only if they can work the complete recurring schedule.

• Provide nursing services in accordance with each person’s plan of care, physician orders, organizational policies, and applicable requirements.
• Administer medications and treatments safely and document services accurately in the medication administration record and other required records.
• Monitor each person’s health status, identify changes or concerns, and take appropriate action, including timely notification and escalation.
• Complete nursing notes, health assessments, incident documentation, medication reviews, and other required records within established timeframes.
• Coordinate with physicians, pharmacies, hospitals, families, direct support professionals, managers, and other members of the interdisciplinary team.
• Support continuity of care during admissions, discharges, appointments, transitions, and changes in medication or treatment orders.
• Provide guidance to direct support staff regarding health protocols, medication-related concerns, and person-specific nursing instructions within the LPN scope of practice.
• Follow infection-prevention, confidentiality, safety, and emergency-response procedures and report suspected abuse, neglect, exploitation, or other reportable events as required.
• Maintain professional conduct and dependable attendance for the complete assigned shift.

Requirements

Required Qualifications

• Current, active, and unrestricted Licensed Practical Nurse license authorizing practice in the District of Columbia.
• Current CPR and First Aid certification, or ability to obtain required certification within the organization’s established timeframe.
• Completion of all required DC Department on Disability Services DDS Phase Trainings. Applicants who previously completed the trainings must provide acceptable documentation for verification.
• Ability to successfully complete all required background checks and regulatory clearances as a condition of employment and assignment.
• Ability to satisfy all other required pre-employment, health, credentialing, and regulatory requirements.
• Working knowledge of medication administration, nursing documentation, infection prevention, and recognition of changes in health status.
• Ability to communicate clearly, maintain confidential information, use electronic documentation systems, and work effectively with an interdisciplinary team.
• Reliable transportation and the ability to report consistently to the assigned Washington, DC, work location.

Preferred Qualifications

• Experience supporting people with intellectual or developmental disabilities in residential, community-based, long-term care, behavioral health, or related settings.
• Experience with electronic medication administration records and person-centered plans of care.
