About the Role

The Licensed Practical Nurse (LPN) – Wound Care is a vital member of the clinical team, responsible for delivering high-quality wound care services in accordance with established protocols and standards. This role supports physicians and patients through clinical procedures, documentation, supply management, and patient education, ensuring optimal outcomes and continuity of care. After successfully completing initial onboarding, the Wound LPN is expected to perform the duties outlined in their new hire training and competency checklist.

Responsibilities
  • Completes ongoing education and competencies as assigned in learning management system prior to due date.
  • Obtains and maintains wound supply certifications, as required.
  • Adheres to administrative and clinical standard operating procedures.
  • Maintains flexibility with given schedule and floats to clinics other than home location as requested by supervisor.
  • When not actively engaged with a scheduled patient, assists other staff members with their duties in the spirit of teamwork to ensure effective and efficient clinic operation.
  • Drives efficient patient flow: uses eCW and/or Nethealth to update patient status throughout visit, promptly rooms patients, performs Patient Verification process, obtains and documents vital signs, takes patient photos, and turns over patient rooms.
  • Ensures procedure rooms are stocked appropriately and communicates with supply chain team as needed.
  • Submits inventory reconciliation form on dates as directed by supply chain team.
  • Adheres to CDC and OSHA standards in wearing appropriate PPE, cleaning, and preparing rooms for procedures.
  • Documents patient’s history of present illness (HPI), preventative screenings, and additional patient information in patient’s medical record, as directed by physician.
  • Completes consent forms accurately prior to all patient procedures and obtains patient signatures.
  • Supports providers in wound care and overall patient case management.
  • Actively participates in Time Out Process for wound procedures.
  • Sets up exam rooms for sterile and nonsterile patient procedures and assists in surgical procedures.
  • Manages and maintains the safety of the Autoclave and ultrasonic machines.
  • Ensures proper cleaning and sterilization and packaging of tools and equipment needed for wound procedures.
  • Performs monthly inventory counts of all procedural tools.
  • Obtains lab cultures from multiple portals and maintains documentation for specimen logs.
  • Notifies patients within their scope of practice of diagnostic testing results guided by wound physician.
  • Maintains proper bandaging techniques as ordered by the provider.
  • Assists in addressing patients' issues, concerns, and handling telephone encounters as assigned.
  • Ensures delivery of ordered wound care supplies to patients’ home, and interacts with DME company.
  • Assists physician with medication preparation.
  • Manages prior authorizations for specialized medications.
  • Maintains documentation requirements and completes patient assistance forms as needed based on insurance coverage.
  • Coordinates with the pharmacy for medications.
  • Facilitates P2P for denials and appeals.
  • Follows up with patients regarding whether specialized medication has been received.
  • Maintains stock of medications in clinic.
  • Maintains ready wound vac kits in the clinic: inventory, expiration dates, and documentation as needed.
  • Graft Management: maintains authorizations, documentation, denials and appeals for each graft.
  • Coordinates with PCP as needed.
  • Documents graft applications that are applied to each patient outside of Net Health and coordinates with the ENVI/Supply Team for billing.
  • Obtains additional graft authorizations as needed.
  • Manages physical storage and temperature control for each graft.
  • Manages Lymphedema Pumps: follows up on patients' delivery of pumps.
  • Documents and maintains compression pump logs, machines and supplies.
  • Completes measurements and forms for custom/ready wear garments.
  • Responsible for medication and supply expiration dates, using FIFO, ensuring items with earliest expiration dates are used first.
  • Demonstrates and promotes a work culture committed to UVVC core values: Understanding, Nurturing, Ingenuity, Trust, Excellence, and Diversity.
  • Demonstrates behaviors consistent with UVVC’s Standards of Conduct as outlined in the Employee Handbook.
  • Maintains the confidentiality and security of PHI in accordance with UVVC policies, HIPAA, and other applicable laws and regulations.
  • Other related duties as assigned.
Qualifications
  • Current and active LPN licensure in the State of Florida; previous wound experience a plus.
  • Minimum of one year of experience as an LPN in a patient care setting.
  • Current Basic Life Support for Healthcare Provider (AHA) or the ability to obtain within 60 days.
  • Excellent verbal and written communication skills.
  • Strong computer skills and experience with electronic medical records.
  • Familiar with medical terminology.
  • Highly dependable; able to meet reliable attendance and punctuality standards.
