Interim HealthCare Inc. in Le Grand, CA is seeking a Home Health Licensed Practical Nurse (LPN) to provide in-home patient care under RN supervision with a flexible schedule that supports work-life balance.

The role includes taking vitals, administering care plans, educating families and coordinating with an interdisciplinary team while enjoying 1:1 nurse-to-patient ratios, online training, and benefits such as sick pay and a 401k option.

Qualifications

• Graduate of an accredited practical nursing program.
• Active LPN license in California.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Minimum of 1 year preferred.
• Clinical proficiency in nursing and strong communication skills.

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
• Educate family on the patient’s plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care.
