The Westchester Center for Rehabilitation & Nursing is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our weekend team. The role requires working every weekend or every other weekend.

Responsibilities

• Provide routine nursing care to patients, including vital sign monitoring, medication administration, and wound care.
• Assist patients with daily living activities such as bathing, dressing, and grooming.
• Maintain accurate and up-to-date patient records, including medication administration records and progress notes.
• Collaborate with therapists and physicians to develop and implement personalized care plans.
• Participate in Quality Improvement initiatives to enhance patient outcomes and satisfaction.
• Contribute to a positive and inclusive work environment through excellent communication and teamwork.
