Sarah Neuman Center for Rehabilitation and Nursing in Mamaroneck, NY invites an LPN to join weekend shifts at our 301-bed facility. You will provide direct nursing care, coordinate with the interdisciplinary team, and support admissions, discharges, and transfers.

Qualifications include a state LPN license and CPR certification; two years of nursing experience is preferred. We offer benefits such as medical, dental, vision, PTO and a 401(k) plan, with Daily Pay available.

Responsibilities

• Admissions, discharge and transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Follow infection control, emergencies, and center policies.
• Collaborate with the interdisciplinary team for optimal resident outcomes.
• Identify and report abnormal changes in residents’ condition per center policy.
• Supervise care under state Nurse Practice Act and facility procedures.
• Deliver nursing care to residents requiring long-term or rehabilitative care.
• Collect and document patient data, observations, and responses.
• Implement the patient plan of care as per regulations.
• Direct and supervise care by other nursing staff in specific situations.
• Maintain documentation and equipment knowledge.
• Uphold confidentiality and resident rights.

Pay Rate: Up to $42/hr

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergencies, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.
• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.
• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

• LPN (Licensed Practical Nurse) licensed by the State
• CPR Certification required
• Two Years prior nursing experience preferred
