Interim HealthCare Home Health is seeking a Licensed Practical Nurse (LPN) Visit Nurse to provide in-home care across Marion and Marysville, OH. You will perform assessments, wound care, and patient education according to care plans, delivering compassionate, patient-centered service.

Qualifications

• Graduation from an accredited practical/vocational nursing program.
• Current, unrestricted LPN license in the state of Ohio.
• Current CPR/AED/BLS/First Aid certification.
• Nursing experience in home health or similar setting preferred.

Responsibilities

• Provide in-home direct patient care per plan of care and nursing scope.
• Complete skilled nursing assessments and patient-centered care in the home.
• Perform wound care and monitor wounds for changes or complications.
• Evaluate patient needs and implement the appropriate plan of care.

As a Licensed Practical Nurse (LPN) Visit Nurse, you will be called to care when you're needed most. As part of Interim HealthCare Home Health, you'll support a full range of patient services to bring comfort and dignity to our clients.

As a Licensed Practical Nurse (LPN) Visit Nurse, you will:

• Provide in‑home direct patient care according to the provider's prescribed plan of care and nursing scope of practice while closely monitoring patient progress and recovery.

• Complete skilled nursing assessments and provide exceptional patient‑centered care in the home.

• Perform wound care and monitor wounds for changes or signs of complications.

• Evaluate patient needs and implement the appropriate plan of care.

• Communicate changes in patient conditions and collaborate with appropriate providers to ensure continuity of care.

• Complete all clinical documentation in accordance with agency protocols and Medicare/Federal guidelines.

• Provide education to patients and family members involved in the patient's care.

• Help decrease readmissions by promoting preventative care and ensuring continuity of care.

To qualify for a Licensed Practical Nurse (LPN) position with us, you will need:

• Graduation from an accredited practical/vocational nursing program.

• Current, unrestricted LPN license in the state associated with this position.

• Current CPR/AED/BLS/First Aid certification.

• Strong nursing assessment and wound care skills.

• A few years of nursing experience in a nursing home, hospital, home health, or similar clinical setting. Home health experience is preferred but not required.

• Reliable transportation to and from patient care locations.

Position Details:

• Pay: $30+ per visit

• Schedule: Approximately 10–20 visits per week to start
