Job summary

Interim HealthCare Inc. is seeking a Licensed Practical Nurse (LPN) to provide in-home care for patients, delivering skilled assessments and a comprehensive care plan. The role emphasizes patient-centered care, collaboration with providers, and accurate documentation in alignment with Medicare guidelines.

The position supports flexible scheduling (FT, PT, PRN), no overtime requirement, and a focus on 1:1 patient care, with opportunities for daily pay and competitive compensation.

Qualifications

• Graduate of a practical nursing program for LPN/LVN with a current license in the practicing state(s).
• At least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program.
• Current unrestricted LPN/LVN licensure in the relevant state.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites and/or work locations.
• Two (2) years of experience practicing as an LPN in home health or similar setting.

Responsibilities

• Provide in-home direct patient care according to the plan of care and nursing scope of practice.
• Complete skilled assessments and deliver patient-centered home care.
• Ensure accurate evaluation and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and collaborate with providers to adapt care as needs change.
• Complete all clinical documentation per agency protocol and Medicare guidelines.
• Educate patients and family members involved in patient’s care.
• Work to decrease readmissions by promoting preventative care and continuity of care.

• Providein-homedirect patient care according to theprovider’sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery
• Complete skilled assessments and provide exceptional patient-centered care in the home

To qualify for a Licensed Practical Nurse (LPN) position with us, you will need:

• Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as an LPN/LVN in the state(s) in which he/she practices and have at least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
• Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
