Job summary

RiverSpring Licensed Home Care Services Agency in Bronx, NY is seeking an LPN Supervisor for a multi-setting clinical role on an immediate per diem basis. The role provides supervision to nursing aides across assisted living and skilled facilities, ensuring quality care and adherence to clinical standards.

You will supervise CNAs/HHCAs, develop and evaluate care plans, document changes, and coordinate care with the interdisciplinary team, while maintaining regulatory compliance and ongoing staff

Qualifications

• Graduate of an accredited school of practical nursing with a current NYS LPN license.
• Minimum 6 months’ experience in long-term care or assisted living preferred.
• Supervisory experience is a plus but not required.
• Strong communication, organizational, and documentation skills; good computer skills.

Responsibilities

• Supervise Certified Nursing Assistants and Home Health Aides; coach, guide, and coordinate shift workflow across settings.
• Assist in developing, implementing, and evaluating individualized nursing care plans.
• Observe, report, and document symptoms and changes to the clinical leader or provider.
• Administer medications and treatments within licensure scope and document.
• Participate in assessments to determine clinical status and service needs.
• Maintain accurate clinical records and ensure regulatory-compliant documentation.
• Collaborate with the interdisciplinary team to support care continuity and service excellence.
• Communicate with providers to secure orders and clarify care plans.
• Maintain current clinical competency and provide staff training as directed.

RiverSpring Licensed Home Care Services Agency is seeking a Licensed Practical Nurse (LPN) Supervisor – Multi-Setting Clinical Role to join our dynamic team!

The RiverSpring Living campus is set on 32 acres overlooking the Hudson River, in Riverdale, New York.RiverSpring Living provides a full range of care solutions including nursing home care, assisted living programs, senior housing, rehabilitation services and specialized services such as elder abuse prevention and memory care.

Position Summary

Provides support to healthcare staff across the Assisted Living Residence and Skilled Nursing Facility as needed. Ensures adherence to clinical standards, quality practices, documentation requirements, workflow coordination, and excellence in patient and resident centered service.

Key Responsibilities

• Supervise Certified Nursing Assistants and Home Health Aides, provide coaching and corrective guidance, ensure quality care, and support shift workflow across settings.
• Assist in developing, implementing, and evaluating individualized nursing care plans based on resident/patient
• Observe, record, and report symptoms, reactions, and changes in condition to the appropriate clinical leader or provider.
• Administer medications and treatments within scope of licensure and document accurately.
• Participate in assessments and reassessments to determine clinical status, risk, and service needs.
• Maintain accurate clinical records, prepare written/oral reports, and ensure documentation supports quality and regulatory compliance.
• Collaborate with interdisciplinary team members to support continuity of care, workflow coordination, and service excellence.
• Communicate with providers to secure medical orders, clarify care plans, report changes, and address medication concerns.
• Maintain current clinical competency, participate in in-service education, and provide training to paraprofessional staff when directed.
