Easterseals Florida is seeking a Licensed Practical Nurse (LPN) as an as-needed Substitute Nurse at our Adult Day Health Care Center in Winter Park, FL. This role offers a consistent Monday–Friday schedule with no nights, weekends, or holidays, and provides meaningful, person-centered care to adults with disabilities.

Responsibilities include health monitoring, nursing support, patient education, and coordinating with families and staff to ensure dignity, independence, and quality of life.

Qualifications

• Graduate of an accredited nursing program.
• Current Florida LPN license in good standing.
• Minimum one year experience with older adults or memory care population.

In this role, you will provide health monitoring, nursing support, participant education, and clinical guidance to ensure participants receive exceptional care while maintaining their dignity, independence, and quality of life.

What You'll Do

• Monitor participant health status and provide nursing services in accordance with individual care plans
• Assist with participant admissions and discharges, including maintaining accurate medical records and required documentation
• Coordinate medical appointments, specialty visits, and transportation needs
• Track, review, and assist with quarterly care plan meetings involving participants and their family/caregiver support teams
• Perform health assessments, document findings, and report concerns, trends, or changes in participant condition
• Provide clinical support, guidance, and training to program staff
• Educate participants and caregivers on health and wellness topics, diagnoses, medications, safety practices, community resources, and emergency preparedness
• Collaborate with interdisciplinary team members to ensure high-quality participant care

Qualifications

• Graduate of an accredited nursing program
• Current Florida Licensed Practical Nurse (LPN) license in good standing
• Minimum one (1) year of experience working with older adults, individuals with disabilities, memory care populations, or individuals with special needs
• Ability to safely assist participants with transfers and activities of daily living, including toileting, bathing, feeding, ambulation, dressing, and other personal care needs
• Strong communication, documentation, and organizational skills
• CPR and First Aid certification preferred (or willingness to obtain upon hire)
