Easterseals Florida is seeking a compassionate LPN for our Adult Day Healthcare program in Gainesville. The role focuses on health monitoring, nursing services, and patient-centered care for about 25 daily participants.

You will support admissions, coordinate visits, and maintain accurate medical records. Qualifications include an accredited nursing program graduate and an active Florida LPN license, with at least one year serving individuals with disabilities or aging populations.

Competenze

• Graduate of an accredited nursing program.
• Current Florida Licensed Practical Nurse License.
• At least one year of experience working with individuals with disabilities and special needs and/or the aging population and Alzheimer's patients.
• Ability to assist with transfers and activities of daily living.
• CPR and first aid certification a plus, but can be obtained/renewed once hired.

• Assist in coordinating admissions and discharges, building new participant charts/medical records, ensuring admissions paperwork is completed.
• Assist in scheduling in-house medical specialty visits, medical/health appointments, and transportation as needed
• Track, review, and coordinate quarterly care plan meetings with participants and family/caregiver team
• Take, document, and maintain records for each participant, reporting trends outside of parameters, or needing attention from the Center Director
• Provide clinical support, insight, knowledge, and applicable in-service training to program staff.
• Provide education to participants and family/caregiver team on health and wellness, medical diagnosis', medication, safe living, emergency situations, and resources available.

Easterseals Florida is seeking a compassionate, professional and dynamic Nurse (LPN) to join our team in our Adult Day Healthcare program in Gainesville. With an average daily attendance of 25 clients, you'll be part of a caring, fun environment. In this position, you will perform all health monitoring and nursing services for participants in the program. The nurse provides excellent care and customer service in a person-centered setting designed to enhance the quality of life for our participants.

Qualifications:

• Ability to assist with the transfer of participants to and from chair or wheelchair and assist clients with activities of daily living (toileting, bathing, ambulating, feeding, changing clothes, etc)
