A health services agency in New York is currently seeking a Licensed Practical Nurse (LPN) to join their Outpatient team in Richmond Hill. In this role, the Outpatient LPN will be responsible for providing direct nursing care to agency clients in accordance with agency policies and procedures and current standards.
About the Opportunity
Start Date: ASAP
Setting: Outpatient Clinic
Schedule / Hours: Monday - Friday & Sundays
Patient Population: Young adults ages 18 to 35 AND Adults ages 36 to 64
Responsibilities:
Assists with patient care including checking vital signs, obtaining other specimens for laboratory analysis and culture as ordered by the medical provider, discussing chief health concerns, and administering injections and immunizations.
Observes, measures, records, and reports data relating to patient mental and physical status.
Administers methadone under the supervision of the RN.
Prepares labels and take-home bottles of medication for self-administration by clients for periods when the medical clinic is closed or for clients who are permitted to self-medicate in accordance with agency policies.
Records and reports any spillage that may occur while dispensing a patient's medication.
Participates in the ongoing assessment of patient's needs.
Follows protocols for methadone dispensing as established by OASAS.
Tallies medication sheet at the end of the day.
Under the supervision of an RN physically counts medication at the end of day closing.
Qualifications;
1+ year of Nursing experience
High School Diploma or GED
Graduate of a formal Technical or Vocational LPN Program
NYS Licensed Practical Nurse (LPN)
BLS certification
At least One (1) year experience as an LPN working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI) or in a similar working environment.
Proficiency in Phlebotomy and EKG.
Methadone experience
Computer savvy; Computer literacy including proficiency in Microsoft Office Suite and EHR.
Knowledge of Medical terminology
Available to work a flexible schedule as needed in response to participant and staff needs.
Desired Skills:
2+ years Nursing Experience
Associates Degree in Nursing
Knowledge of multi-disciplinary team experience
Knowledge of techniques for identifying and preventing potentially violent behavior, including Crisis Management techniques
