Care Options for Kids in Pennsylvania is seeking a Licensed Practical Nurse (LPN) for pediatric home health coverage across Philadelphia County. This per diem role focuses on one-on-one patient care, with vent/trach cases and RN supervision, emphasizing stability and professional growth.

You’ll collaborate with a clinical leadership team, maintain CDC-aligned safety practices, and access benefits such as PTO, 401(k), weekly pay, and ongoing training.

Qualifications

• Valid PA LPN license is required.
• TB skin test or TB blood test is required.
• Valid BLS CPR cert (in-person) required.

Responsibilities

• Provide one-on-one pediatric home health care in patients' homes.
• Support care coordination with RN and clinical leadership.
• Deliver skilled nursing care for vent/trach patients as assigned.
• Follow plans of care with RN oversight and CDC safety guidelines.
• Communicate with families and document care activities.

Education

Valid Pennsylvania LPN License or Multistate License

Float between various school districts within Philadelphia County. Providing coverage for school nurses during absences, sick days, and time off. Per diem schedule with flexible hours ranging from 8 to 40 hours per week.

Requirements

• Valid Pennsylvania LPN License or Multistate License
• TB Skin Test (PPD) or TB Blood Test (QF)
• Valid BLS CPR card (obtained in person not online)
• Valid driver’s license
• G-tube, trach, vent experience or willing to train
