Job summary

Signature HealthCARE Fentress Co. is seeking an LPN/LVN to provide direct nursing care in a 140-bed skilled nursing facility in Jamestown, TN. The role includes medication administration, wound care, and supervising nursing assistants, within a resident-centered care model.

Join a family-based organization focused on quality of life, professional development, and teamwork. Sign-on bonuses of $7,000 for day shifts and $10,000 for night shifts are available.

Qualifications

• Current active LPN/LVN license in the state of practice.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Supervisory or charge nurse experience preferred.
• CPR/BLS certification preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) while ensuring dignity, comfort and safety.
• Supervise nursing assistants and medication aides during assigned shifts.
• Monitor residents’ status and communicate changes to physicians and families.

Additional Details :

$7,000 Sign On Bonus Available for Days

$10,000 Sign On Bonus Available for Nights

How you Will make a Difference :

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference :

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
