Job summary

Signature HealthCARE is seeking a dedicated LPN/LVN to join our team at Signature HealthCARE of Fentress County. This role provides direct nursing care, administers medications, and supports residents with daily activities.

The ideal candidate holds an active LPN/LVN license, has at least one year of clinical nursing experience, and can supervise nursing assistants. Proficiency with EHR systems and good communication are essential.

Qualifications

• Active LPN/LVN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role.
• Strong documentation and medication administration skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living and ensure comfort, dignity and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' health status and notify physician of changes.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers and discharges.
• Participate in quality assurance and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.

How you Will make a difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
