Job summary

Signature HealthCARE LLC is seeking a dedicated LPN/LVN to join our team in a skilled nursing setting. You will provide direct patient care, administer medications, and support residents with daily activities while ensuring safety and dignity.

The role includes supervising aides, documenting care, and collaborating with an interdisciplinary team to update care plans. Experience in long-term care and familiarity with EHR systems are valued.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living while ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides per shift and facility policy.
• Monitor residents’ status and report changes to physicians and families as needed.
• Develop, implement, and review individualized care plans with the care team.
• Complete accurate clinical documentation per policy and procedure.
• Coordinate admissions, transfers, and discharges per established procedures.
• Participate in quality assurance activities and staff education.
• Perform other duties as assigned by the Director of Nursing or designee.

How you Will make a difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
