Job summary

Artis Senior Living of Wilmette seeks a dedicated LPN/LVN to join our care team as Coordinator of Health & Wellness. You will monitor residents with dementia, support restorative and daily care, and work under physician orders to ensure safe, compassionate service.

You will supervise CNAs, manage medication programs, document observations, assist with move-ins, and engage families; state guidelines are followed. Full-time roles with rotating shifts in a Great Place to Work environment.

Qualifications

• Must possess current LPN or LVN designation as granted by State Board of Nursing.
• Continuing education credits as required by state.

Responsibilities

• Provide clinical monitoring and support for residents with Alzheimer's/Dementia.
• Oversee and implement the self-administered medication management program for residents.
• Administer medication and/or treatment to residents as ordered by physicians.
• Document and report care provided and resident observations including changes in conditions.
• Provide emotional support and ensure safety of residents.
• Supervise CNAs and Caregivers in accordance with guidelines.
• Assist with resident move-ins, relocations, and acclimation to surroundings.
• Support marketing efforts and community relationships as needed.

*We have full-time openings, with the following shift availability: 7am-3pm and every other weekend, 3pm-11pm and every other weekend, and 11pm-7am and every other weekend!

How You'll Contribute

We proudly title our LPNs as Coordinators of Health & Wellness, reflecting the collaborative role they play in our community and the important work they do to support our residents and families.

The Coordinator of Health & Wellness will provide clinical monitoring and support for residents with Alzheimer's/Dementia. Ensure that all restorative, support and preventative measures are met in accordance with applicable state laws and regulations. Experience with Alzheimer's/Dementia or behavioral health conditions, and a willingness to work with the elderly population are essential for this role.

The Coordinator of Health & Wellness will:

• Take an active role in resident assessments, development of service plans, and daily care.
• Oversee and implement the self-administered medication management program for residents.
• Administer medication and/or treatment to residents in accordance with physician orders.
• Document and report care provided and resident observations including unusual or significant changes in physical, mental, or behavioral conditions.
• Provide emotional support, physical comfort and ensure ongoing safety of residents.
• Supervise Caregivers and CNAs in providing resident care in accordance with State, Federal and Artis guidelines.
• Ensure residents receive the highest quality of service in a caring and compassionate atmosphere which recognizes their individual needs and rights.
• Assist in the marketing of the community by touring and participating in public relation events, establishing relationships with potential resident referral sources.
• Assist with resident move-ins and relocations, help residents acclimate to their new surroundings and community personnel.
• Performs other duties as required.

How You'll Succeed

• Must possess current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) designation as granted by State Board of Nursing.
• Fulfill and maintain continuing education credits as required by state.
• Experience in rehabilitative or geriatric nursing is preferred.
