Job summary

Artis Senior Living in Olney, MD is hiring a Coordinat*or of Health & Wellness* (LPN/LVN) to provide clinical monitoring and support for residents with Alzheimer's/Dementia. The role emphasizes patient safety, medication management, and collaborative care with a compassionate team.

Qualifications

• Must possess current LPN/LVN designation as granted by State Board of Nursing.
• Continuing education credits must be fulfilled/maintained as required by state.
• Experience in rehabilitative or geriatric nursing is preferred.

Responsibilities

• Take an active role in resident assessments and development of service plans.
• Oversee and implement self-administered medication management for residents.
• Administer medications and treatments per physician orders.
• Document and report care provided and any significant resident changes.
• Provide emotional support and ensure resident safety.
• Supervise Caregivers and CNAs in delivering care.
• Assist with marketing and community relationships with referral sources.
• Support resident move-ins and acclimation to their new surroundings.

*Starting pay is $32 / hour and up based on experience!

*We have Part-Time and PRN openings, with the following shift availability: 7am-3pm and every other weekend, 3pm-11pm and every other weekend, and 11pm-7am and every other weekend!

How You'll Contribute

We proudly title our LPNs as Coordinators of Health & Wellness, reflecting the collaborative role they play in our community and the important work they do to support our residents and families.

The Coordinator of Health & Wellness will provide clinical monitoring and support for residents with Alzheimer's/Dementia. Ensure that all restorative, support and preventative measures are met in accordance with applicable state laws and regulations. Experience with Alzheimer's/Dementia or behavioral health conditions, and a willingness to work with the elderly population are essential for this role.

The Coordinator of Health & Wellness will:

• Take an active role in resident assessments, development of service plans, and daily care.
• Oversee and implement the self-administered medication management program for residents.
• Administer medication and/or treatment to residents in accordance with physician orders.
• Document and report care provided and resident observations including unusual or significant changes in physical, mental, or behavioral conditions.
• Provide emotional support, physical comfort and ensure ongoing safety of residents.
• Supervise Caregivers and CNAs in providing resident care in accordance with State, Federal and Artis guidelines.
• Ensure residents receive the highest quality of service in a caring and compassionate atmosphere which recognizes their individual needs and rights.
• Assist in the marketing of the community by touring and participating in public relation events, establishing relationships with potential resident referral sources.
• Assist with resident move-ins and relocations, help residents acclimate to their new surroundings and community personnel.
• Performs other duties as required.

How You'll Succeed

• Must possess current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) designation as granted by State Board of Nursing.
• Fulfill and maintain continuing education credits as required by state.
• Experience in rehabilitative or geriatric nursing is preferred.
