Job summary

Artis Senior Living in Eatontown, NJ is hiring a Coordinating Health & Wellness LPN/LVN for a full-time 2nd shift (3pm-11pm). Starting pay is $35/hour with every-other-weekend requirements.

You will supervise Caregivers and CNAs while delivering clinical care to residents with Alzheimer's/dementia and ensuring safety and dignity in daily care. We offer competitive medical, dental and vision plans, paid holidays, 3 weeks PTO, 401k with employer match, and tuition reimbursement.

Qualifications

• Must possess current LPN or LVN designation as granted by State Board of Nursing.
• Experience in rehabilitative or geriatric nursing is preferred.

Responsibilities

• Provide clinical monitoring and support for residents with Alzheimer's/Dementia.
• Oversee self-administered medication management and administer medications per physician orders.
• Document and report care provided and notable changes in residents' conditions.
• Supervise Caregivers and CNAs to ensure quality resident care.
• Support residents and families with emotional well-being and safety.
• Assist with move-ins/relocations and participate in community events.

How You'll Contribute

We proudly title our LPNs as Coordinators of Health & Wellness, reflecting the collaborative role they play in our community and the important work they do to support our residents and families.

The Coordinator of Health & Wellness will provide clinical monitoring and support for residents with Alzheimer's/Dementia. Ensure that all restorative, support and preventative measures are met in accordance with applicable state laws and regulations. Experience with Alzheimer's/Dementia or behavioral health conditions, and a willingness to work with the elderly population are essential for this role.

The Coordinator of Health & Wellness will:

• Take an active role in resident assessments, development of service plans, and daily care.
• Oversee and implement the self-administered medication management program for residents.
• Administer medication and/or treatment to residents in accordance with physician orders.
• Document and report care provided and resident observations including unusual or significant changes in physical, mental, or behavioral conditions.
• Provide emotional support, physical comfort and ensure ongoing safety of residents.
• Supervise Caregivers and CNAs in providing resident care in accordance with State, Federal and Artis guidelines.
• Ensure residents receive the highest quality of service in a caring and compassionate atmosphere which recognizes their individual needs and rights.
• Assist in the marketing of the community by touring and participating in public relation events, establishing relationships with potential resident referral sources.
• Assist with resident move-ins and relocations, help residents acclimate to their new surroundings and community personnel.
• Performs other duties as required.

How You'll Succeed

• Must possess current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) designation as granted by State Board of Nursing.
• Fulfill and maintain continuing education credits as required by state.
• Experience in rehabilitative or geriatric nursing is preferred.
