Licensed Practical Nurse (LPN) – Rehabilitation

Licensed Practical Nurse joining our team plays a vital role in delivering compassionate, patient‑centered care in a 40‑bed inpatient rehabilitation setting.
What We're Looking For
Responsibilities include:
- Demonstrates knowledge of medications and correct administration based on patient age and clinical condition.
- Reduces the potential for medication errors by following correct medication procedures.
- Performs patient care considering needs specific to the standard of care for patients’ age; includes geriatric and general inpatient rehabilitation patients.
- Under R.N. supervision, assists with the assessment of all patients and reassessments as per policy, identifies and initiates appropriate rehabilitation nursing interventions.
- Under R.N. supervision, performs timely and accurate QI assessments.
- Carries out the plan of care as indicated by the patient’s needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly.
- Interacts professionally with patient/family and involves patient/family in the formation of care plans.
Qualifications and Requirements
- Graduate of an accredited licensed practical or vocational nurse program.
- Current state licensure as Licensed Practical/Vocational Nurse.
- CPR/BCLS certification; ACLS preferred.
- IV Certified required or obtainable within 6 months.
- Minimum six months medical/surgical experience in an acute care setting preferred.
- Excellent oral and written communication and interpersonal skills.
