Job summary

Community West Rehabilitation Hospital in Brownsburg, IN is hiring a Licensed Practical Nurse (LPN) for full-time night shifts. The 36-hour work week comprises two 12-hour shifts from 7:00 p.m.

to 7:30 a.m., working under RN supervision to deliver rehab-focused patient care. Responsibilities include medication administration, patient assessments with age-specific considerations, and contributing to the interdisciplinary care plan.

Qualifications

• Licensed Practical Nurse license in Indiana is required.
• CPR/BCLS certification is required; ACLS preferred.
• Excellent oral and written communication and interpersonal skills.
• familiarity with rehab documentation systems is preferred.

Responsibilities

• Demonstrates knowledge of medications and their correct administration based on the patient’s age and clinical condition.
• Performs patient care considering needs specific to rehab patients (geriatric and general inpatient).
• Under RN supervision, assists with patient assessments and reassessments.
• Under RN supervision, conducts timely and accurate QI assessments.
• Carry out the plan of care and update the interdisciplinary care plan based on patient changes.
• Interact professionally with patient and family and involve them in care planning.
• Assess and reassess pain; educate patient and family on pain management.
• Other duties as assigned.

How you'll contribute

A Licensed Practical provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.

Essential Functions

• Demonstrates knowledge of medications and their correct administration based on the age and clinical condition of the patient
• Performs patient care considering needs specific to the standard of care for patient's age; includes geriatric and general inpatient rehabilitation patients
• Under the supervision of the RN, assists with the assessment on all patients and reassessments as per policy.Identifies and initiates appropriate rehabilitation nursing interventions
• Under the supervision of the RN, performs timely and accurate QI assessments
• Carry out the plan of care as indicated by the patient's needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly based on changes in patient's condition and individual needs
• Interacts professionally with patient/family and involves patient/family in the formation of the plan of care
• Demonstrates ability to adequately assess and reassess pain.Utilizes appropriate pain management techniques.Educates the patient and family regarding pain management
• Other duties as assigned

What we're looking for

Applicants should have a Licensed Practical Nurse license in the state of Indiana. Additional requirements include:

• Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse.
• CPR/BCLS certification. ACLS preferred.
• Excellent oral and written communication and interpersonal skills.
