Job summary

Lifepoint Health is seeking a Part-Time Day shift LPN at the Hospital Sisters Health System Rehabilitation Hospital, a new 40-bed IRF in Wisconsin. You will provide direct and coordinated patient care using the nursing process and collaborate with an interdisciplinary team to deliver high-quality care.

The role requires graduation from a nursing program (BSN/ADN or diploma), current LPN licensure in Wisconsin, and CPR/BCLS certification; ACLS is preferred.

Qualifications

• Graduation from an accredited nursing program (BSN/ADN or diploma).
• Current state licensure as Licensed Practical Nurse.
• CPR/BCLS certification; ACLS preferred.
• Minimum six months medical/surgical experience in acute care.
• Excellent oral and written communication and interpersonal skills.

Responsibilities

• Provides planning and delivery of direct and indirect patient care through the nursing process (Assessment, Planning, Intervention, Evaluation).
• Develops nursing care plans in coordination with patient, family and interdisciplinary staff.
• Communicates changes in patient condition with physicians, nursing supervisor/manager, and co-workers.
• Participates in discharge planning process.

How You'll Contribute

Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plan in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient's clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate. Participates in discharges planning process.

• Deliver direct and indirect patient care using the nursing process-Assessment, Planning, Intervention, and Evaluation
• Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans
• Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues
• Play a key role in the discharge planning process to support safe, successful transitions

What We're Looking For

Applicants should possess the following qualifications and skills:

• Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse.
• CPR/BCLS certification. ACLS preferred.
• Minimum six months' Medical/Surgical experience in an acute care setting preferred.
• Excellent oral and written communication and interpersonal skills.
