Pinnacle Home Care, Florida's largest independent Medicare-certified home health provider, is hiring a Licensed Practical Nurse (LPN) to deliver skilled nursing care in patients' homes. Responsibilities include nursing tasks under RN supervision, accurate documentation, and ensuring quality outcomes.

Requirements include a current LPN/LVN license, valid BLS, and at least one year of nursing experience (home health preferred).

Qualifications

• Must hold a current LPN or LVN license and graduate of an accredited nursing program.
• Valid BLS certification.
• Minimum one year nursing experience, preferably in home health.
• Excellent communication, organization, and time management.
• Valid driver’s license and access to a reliable vehicle.
• Ability to visit homes, some may not be wheelchair accessible.
• Sufficient hearing, eyesight and dexterity to perform assessments.

Responsibilities

• Administer skilled nursing care under RN supervision.
• Perform care tasks per patient care plans (curative, rehabilitative, preventive).
• Use clinical judgment to support patient care and outcomes.
• Complete visit reports promptly and accurately in the EMR with required documentation.
• Confirm daily visit schedules to ensure coordination with staff.
• Participate in the agency’s QAPI program as required.
• Adhere to patient rights and discharge policies.

Key Responsibilities

• Administer skilled nursing care under RN supervision.
• Perform curative, rehabilitative, or preventive care tasks as outlined in patient care plans.
• Utilize clinical expertise and nursing education to make informed, independent decisions that support patient care and promote positive outcomes.
• Complete visit reports promptly and accurately within the electronic medical record system to support high-quality care and regulatory compliance, including all required documentation.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
• Understand and adhere to the agency’s process for managing and addressing customer complaints.
• Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
• Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.

• Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
