Job summary

Signature HealthCARE is seeking an experienced LPN/LVN to provide direct nursing care for residents in a 120-bed facility. You will supervise nursing staff, administer medications, and ensure safety and dignity for residents in a compassionate team environment.

The role requires an active LPN/LVN license in the state of practice, clinical nursing experience, and familiarity with EHR systems. Excellent communication and judgment are essential in daily coordination and documentation.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the assigned shift per policies.
• Monitor residents’ physical and emotional status and notify physician of changes.
• Develop, implement, and review individualized resident care plans with the team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.

How you Will make a difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.

• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
