Job summary

Signature HealthCARE is seeking an LPN/LVN to provide direct nursing care in a skilled nursing setting. You will supervise nursing assistants, manage medication administration, and coordinate resident care plans with the interdisciplinary team.

Ideal candidates have 1+ year of clinical nursing experience, familiarity with EHR, and a strong commitment to quality outcomes in long-term care. On-site role with a supportive team and ongoing professional development.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Supervisory or charge nurse experience preferred.
• Knowledge of Federal and State long-term care regulations.
• Strong communication skills (verbal and written).
• Familiarity with EHR systems and basic office software.

Responsibilities

• Provide direct nursing care to residents, including meds and skilled procedures.
• Assist residents with ADLs, ensuring comfort, dignity and safety.
• Supervise nursing assistants and med aides during shifts.
• Monitor residents' status and report changes to physician/guardian.
• Develop and review individualized care plans with the team.
• Complete timely clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and staff education.

How you Will make a difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
