Position Title: Licensed Practical Nurse (LPN) PRN
Beacon Health Management is hiring a Licensed Practical Nurse (LPN) to work at Hazlehurst Court Care and Rehab providing both short term rehabilitation and long-term care.
Position Summary
In conjunction with the Unit Director, the Licensed Practical Nurse utilizes a general understanding of the principles of Nursing in the development of and implementation of individualized Nursing Care Plans to ensure that the needs of the customers are met. He/she assists in the orientation of and supervision of nursing personnel, attends to the daily operations of the unit on per shift, unit level, and assumes responsibilities of a leadership role as needed.
Licensed Practical Nurse (LPN) RESPONSIBLITIES
- Takes an active role in direct customer care
- Administers medication and/or treatment and documentation of same to customers
- Responsible for clinical documentation related to customer activity
- Assists in developing and implementing individual nursing care plan for each customer. Participates in care plan meetings
- Counts and records class drugs when beginning and ending shift
- Communicates pertinent data to Unit Manger/Supervisor and/or Physician
- Reviews and updates communication board; as well as monitors and addresses alerts frequently throughout shift
- Provides for emotional and physical comfort and safety of customers
- Assist Physician in rounds and examination of customers, as needed
- Supervises and coordinates activities of Certified Nursing personnel and other LPN's in providing direct customer care in adherence to State, Federal and Corporate guidelines
- Enhances education by attending all mandated in-service programs
- Puts Customer Services First: Ensures that customers and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights
Educational/Vocational Requirements
- The Licensed Practical Nurse must be a graduate of an approved school for Practical or Vocational Nursing and must be currently licensed by the State Board of Nursing.
- Experience is preferred in geriatric or rehabilitative Nursing
