Job summary

NorthStar Home Hospice is seeking a Licensed Practical Nurse (LPN) to provide compassionate care for Hospice patients in their homes based in Olde West Chester, Ohio. You will coordinate with colleagues to implement personalized care plans and monitor patient conditions while administering medications. Previous LPN experience is preferred, and a valid Ohio nursing license and CPR certification are required. The position offers opportunities for advancement and competitive compensation.

Job Summary

We are seeking a Licensed Practical Nurse to join our team. In this role, you will be caring for Hospice patients in their homes. You are an LPN with great attention to detail and a big heart. You take pride in caring for patients following personalized care plans. You are able to assess, observe and communicate effectively regarding your patient’s care.

Responsibilities

• Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
• Administer medications and monitor patients for side effects or reactions
• Document any changes in patient conditions and report on changesMaintain accurate records of patient care and discuss observations with the supervisor

Qualifications

• Currently licensed Licensed Practical Nurse (LPN) in state of Ohio
• Previous experience as a Licensed Practical Nurse is preferred
• Hospice experience preferred but not required
• Commitment to excellent care and customer service
• Excellent interpersonal and client care skills
• Valid driver’s license
• First aid/CPR certification required
