Tampa Lakes Health & Rehabilitation Center – Lutz, FL is actively hiring Licensed Practical Nurses (LPNs) for full-time and part-time roles. Eligible candidates hold an active Florida LPN license, IV certification, CPR certification, and have prior nursing experience.

Qualifications

• Active Florida LPN license required.

• Two years prior nursing experience preferred.

Responsibilities

• Admissions, Discharge and Transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Following infection control, emergencies, and other center policies.
• Collaborating with resident’s interdisciplinary team for best outcomes.
• Identifying and reporting abnormal changes in residents’ condition per center policy.
• Works under state-specific Nurse Practice Act and facility Policies.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data and reports pertinent information.
• Implements and evaluates the patient/resident care plan per regulations.

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergencies, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.
• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.
• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

• LPN (Licensed Practical Nurse) licensed by the State or Multistate License
• CPR Certification required
• Two Years prior nursing experience preferred
