We are seeking a Licensed Practical Nurse (LPN) to support the continuous enrichment and development of individuals with developmental disabilities in four (4) of our adult residential facilities in South Bend, IN.

What You Get To Do:
- Ensure that each individual receives quality, individualized health care management, resulting in their optimal health
- Conduct routine site visits, check vitals, perform assessments, counsel on medication administration, and document progress
- Act as a health/medical resource for staff, training staff in the medical arena
- Conduct audits and reviews of medications
- Ensure that the company complies with state requirements and organizational policies regarding medical care and records
Schedule:
- Full-time Monday-Friday with occasional travel required; flexible hours
- Participate in an on-call rotation for triage of medical concerns for 1 week per month
What Makes You A Great Fit:
- Current Licensed Practical Nurse (LPN) license in the State of Indiana required
- 1-2 years experience working with individuals with disabilities or in long-term care preferred
- Skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
- Ability to work independently and efficiently
- Organizational and computer skills
- Acceptable background check & driving record check
