Job summary

Collage Home and Community Rehabilitation Partners is seeking experienced LPNs for PRN positions in Southlake, TX and surrounding areas. You will join interdisciplinary teams delivering patient-centered care in patients’ homes and communities, contributing to integrated treatment plans that address functional goals.

The role supports patients across the cognitive and physical spectrum, with an evidence-based approach designed for durable outcomes.

Qualifications

• One year of LPN experience in a home care setting.
• Current state license as an LPN.
• Graduate of an accredited practical nursing program.
• CPR certification current.
• TB test negative.
• Ability to work flexible schedule.
• Excellent interpersonal skills.
• Valid driver’s license with reliable transportation.
• Willingness to travel locally.

Responsibilities

• Participate in planning and coordination of total patient care with the RN and physician’s plan.
• Report and record patient condition and care changes.
• Prepare clinical/progress notes and summaries.
• Assist patient with activities of daily living and self-care instruction.
• Provide and maintain a safe patient environment.
• Assist physician and RN in complex nursing situations.
• Follow universal precautions during patient care.
• Follow emergency procedures for catastrophic events or significant changes in patient condition.

We are looking for experienced LPN’s who want to provide truly patient-centered care in Southlake, TX and surrounding areas - with PRN positions available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Areas of responsibility include but are not limited to:

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change
• Prepare clinical/progress notes and summaries
• Assist the patient with the activities of daily living while teaching appropriate self-care techniques
• Provides and maintains a safe environment for the patient
• Assist the physician and Registered Nurse in complex nursing situations
• Follow universal precautions whenever giving any aspect of patient care
• Follow emergency procedures in the event of any catastrophic event, incident, e.g., accident, injury or significant change in the patient's condition

Qualifications

• 1 year experience as an LPN in a home care setting
• Current state license as an LPN
• Graduate of an accredited program of practical nursing
• Proof of current CPR certification
• Proof of current negative TB test
• Ability to work flexible schedule
• Excellent interpersonal skills
• Valid driver’s license and reliable transportation
• Ability to work a flexible schedule and travel locally
