Collage Home and Community Rehabilitation Partners is seeking experienced RNs for PRN, casual home-health positions in Southlake, TX and surrounding areas.

Join our patient-centered team to provide one-on-one in-home care, conduct assessments, administer medications, educate patients and families, and help implement durable, functional goals in patients' homes. You will collaborate with therapists and physicians to adapt plans, monitor progress, and ensure quality outcomes.

Qualifikationen

• Graduate of an accredited nursing program (Diploma/Associate/Bachelor)
• Current RN license in Texas
• CPR certification current
• Compassionate, friendly demeanor
• Valid driver's license and reliable transportation
• Ability to work a flexible schedule and travel locally

We are looking for experienced RN’s who want to provide truly patient-centered care in Southlake, TX and surrounding areas - with PRN positions available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.

Areas of responsibility include but are not limited to:

• Visit patients in their homes for one-on-one visits


• Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients


• Assess patients’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review


• Administer medication as prescribed by the patient’s physician


• Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life


• Provide effective communication to patients/clients, their family members, team members, and other health care professionals

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing


• Current state license as an RN


• Proof of current CPR certification


• Compassionate, friendly demeanor


• Current driver’s license and reliable transportation


• Ability to work a flexible schedule and travel locally
