Job summary

Collage Home and Community Rehabilitation Partners in Denver, CO is seeking an experienced Licensed Practical Nurse (LPN) for Neuro-PRN work in patients' homes and communities. You will join interdisciplinary teams delivering patient-centered care with an evidence-based approach.

Pay is $34–$36 per hour. PRN positions offer flexibility to support a range of neurological rehabilitation needs across the Denver area.

Qualifications

• 1 year of LPN home care experience or equivalent.
• Current LPN license in good standing.
• CPR certification and TB test documentation.
• Ability to work flexible scheduling and travel locally.

Responsibilities

• Participate in planning and coordination of total patient care with RN and physician plan of care.
• Accurately report and record patient condition and care changes.
• Prepare clinical notes and summaries.
• Assist with activities of daily living and teach self-care techniques.
• Maintain a safe patient environment and follow universal precautions.
• Support physician and RN in complex nursing situations.
• Follow emergency procedures during catastrophic events.

We provide treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neuro or complex ortho diagnoses in the most functional setting there is - a person's own home and community.

We are looking for experienced LPN’s who want to provide truly patient-centered care in Denver, CO and surrounding areas - with PRN positions available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.

Pay: $34-$36 per hour

Areas of responsibility include but are not limited to:

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change
• Prepare clinical/progress notes and summaries
• Assist the patient with the activities of daily living while teaching appropriate self-care techniques
• Provides and maintains a safe environment for the patient
• Assist the physician and Registered Nurse in complex nursing situations
• Follow universal precautions whenever giving any aspect of patient care
• Follow emergency procedures in the event of any catastrophic event, incident, e.g., accident, injury or significant change in the patient's condition

Qualifications

• 1 year experience as an LPN in a home care setting
• Current state license as an LPN
• Graduate of an accredited program of practical nursing
• Proof of current CPR certificationProof of current negative TB test
• Ability to work flexible schedule
• Excellent interpersonal skills
• Valid driver’s license and reliable transportation
• Ability to work a flexible schedule and travel locally
