Qualifications

• Active LPN license in Indiana or compact license.
• Minimum 1–2 years of clinical nursing experience (home health and pediatric preferred).
• Experience with G-tube, trach, or vent care is a plus (training provided).
• Current CPR certification.
• Strong critical thinking and confident independent decision-making.
• Outstanding interpersonal and communication skills; reliable transportation and a valid driver’s license.

Responsibilities

• Conduct comprehensive head-to-toe assessments and carry out personalized care plans.
• Deliver hands-on care such as G-tube feedings, medication administration, wound care, trach care, and ventilator support.

Altra Home Health is looking for a caring and skilled Licensed Practical Nurse (LPN) to become part of our team. In this rewarding position, you'll deliver personalized, top-notch care to pediatric and adult patients in the comfort of their own homes. Many of our patients face complex medical challenges, including gastrostomy tubes (G-tubes), cerebral palsy, chronic conditions, and occasionally tracheostomy or ventilator support. You'll be instrumental in fostering safety, comfort, and quality of life—empowering patients to receive the care they need right where they feel most comfortable: home.

Responsibilities
