Job summary

Care Advantage, Inc. is seeking licensed practical nurses (LPN) for home health care in the Virginia Beach area. The role runs 8 AM to 4 PM, Monday through Friday, providing one-on-one care in clients' homes to improve mood, cognitive well-being, and comfort.

Care Advantage, Inc. is looking for multiple Licensed Practical Nurses (LPN) in the Virginia Beach, VA area who are interested in working an 8 AM to 4 PM case running Monday through Friday. Our care teams provide one-on-one care in clients' homes to provide the best care possible to our patients. Our goal is to decrease depression and mitigate cognitive impairment with specialized care plans developed by our Registered Nurses while keeping our patients safe and comfortable in their own homes. LPN Responsibilities:

• Provide and implement direct patient care
• Provide accurate and timely documentation consistent with the plan of care
• Assess and provide family/caregiver education and information
• Assist the RN or physician in performing specialized procedures and duties, which include preparing equipment and materials for treatments, observing aseptic technique as required, and assisting the patient in learning appropriate self-care techniques
• Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor

Requirement/Eligibility:

• Eligible to work in the United States
• CPR Certification and PPD Certification
• 1+ year experience as an LPN
• Valid LPN License with at least (1) one year of work experience
• Valid Driver's license
• Reliable transportation
