Job summary

Starr Regional Medical Center is seeking a Long Term Care LPN for per diem/PRN rotations. You will work under an RN to assess patients, perform care plans, and assist with daily living activities in a team-focused LTC unit.

This role emphasizes compassionate care, patient advocacy, and collaboration with families. Eligible candidates must have an LPN license and BCLS, with potential weekend rotations.

Qualifications

  • Graduate of an LPN program is required.
  • BCLS certification within 30 days of hire/transfer.

Responsibilities

  • Under the direction of an RN, contributes to patient assessment by collecting data.
  • Performs plan of care interventions including wound care and specimen collection; may administer medication.
  • Assists RN with development of the plan of care.
  • Assists patients with activities of daily living and maintains patient hygiene and nutrition.
  • Monitors and communicates patient status and reactions to interventions.

How you'll contribute

ALong Term Care LPNwho excels in this role:

  • Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN.
  • Performs plan of care interventions, including specimen collection and wound care. May administer medication. Documents patient care given.
  • Assists RN with development of plan of care.
  • Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
  • Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
  • Works as an advocate for the physical and emotional well-being of the patient.

What we're looking for
  • Education: Graduate of an LPN program
  • Required Skills: Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
  • Certifications: BCLS within 30 days of hire/transfer
  • Licenses: Active TennesseeLPN license (single-state or compact) required. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins.
