Qualifikationen

• Current Washington State LPN license.
• Current BLS certification.
• Hospice, home health, or palliative care experience preferred.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and interpersonal skills.
• Valid driver's license and reliable transportation.

Aufgaben

• Provide skilled nursing care to hospice patients in accordance with physician orders, hospice policies, and regulatory standards.
• Administer medications, monitor patient conditions, and document care accurately.
• Support symptom management and patient comfort.
• Educate patients and families about medications, comfort measures, and the plan of care.
• Collaborate with RNs, hospice aides, social workers, chaplains, and other members of the hospice care team.
• Respond promptly to patient and family needs, including symptom management and urgent care situations.
• Maintain accurate documentation and follow hospice policies, state regulations, and Medicare guidelines.
• Participate in the weekend on-call rotation as scheduled.
