Job summary

Interim HealthCare of Collinsville, VA is seeking a compassionate Home Health LPN to provide one-on-one patient care in Collinsville and surrounding areas. You will work independently in patients’ homes, under RN supervision, delivering skilled nursing services and implementing care plans.

This part-time/PRN position offers flexible scheduling, mileage reimbursement, and opportunities for online training, with a supportive team and a focus on improving patient outcomes.

Qualifications

• Graduate of an accredited practical nursing program.
• Active LPN license in Virginia.
• Minimum of 1 year of LPN experience (home health preferred) or completion of office preceptor program.
• CPR certification and TB skin test required.
• Knowledge of state and federal home health regulations.
• Strong clinical judgment, communication, and critical thinking skills.
• Compassionate, dependable, and patient-focused.
• Ability to work independently in a home setting.

Responsibilities

• Provide home-based nursing care to patients recovering from illness, injury, or chronic conditions under the supervision of an RN.
• Monitor patient condition, obtain vital signs, and perform routine nursing assessments.
• Document patient progress and report changes in condition to RN supervisor.
• Assist with implementing individualized patient care plans.
• Educate patients and families on medications, treatment plans, self-care, and home safety.
• Collaborate with physicians, RNs, CNAs, aides, and interdisciplinary team members to coordinate patient care.
• Assist with discharge planning and ensure continuity of care.
• Maintain accurate and timely documentation.
